Person-to-Person Mentor Program

The goal of the Person-to-Person Mentor Program is to connect people who are newly diagnosed with Parkinson disease to others currently living with Parkinson’s disease. The idea is that the two will support each other one-on-one through difficult medication changes, and help each other through everyday life. Communication may take the form of face-to-face meetings, email, U.S. mail or telephone – your choice. To assist us in finding the best possible match for you, please fill out this form and email, fax or mail it to the
American Parkinson Disease Association Information & Referral Center,
700 S. Park Street,
WI 53703.
